The fee for a missed continuing patient appointment is $50.  By scheduling an appointment with our online booking service, you accept and agree to the relevant cancellation and no-show policy provided. 

In order to complete your booking, please check your email for a verification message from Full Slate after you have pressed the "Schedule Appointment" button below. Once you click the verification link, your appointment will be scheduled.

You must verify your email within 10 minutes or you'll your appointment will not be scheduled.
